The Walkerton & District Hospital Foundation has created a display piece to recognize the babies born in the Dr. RJ Creighton Family Birthing Centre. If you wish to recognize your baby on the ‘Star is Born’ recognition wall, please read the following information and click the ‘purchase star here’ button below to purchase online. Forms can also be printed by clicking the ‘download form here’ button and returned to the Foundation Office in person or via email – wdhfoundation@sbghc.on.ca. Stars are available for purchase for $100 each. A $50 tax receipt will be provided after purchase.

The star will be displayed on the wall for a 1 year term. (Please note that display time frames are approximate and may vary depending on when the wall is updated.) After your term has completed, your star will be returned to you via mail or can be picked up at the Foundation Office.

If you wish to purchase additional stars for previous children, you are welcome to do so. However, only babies born over the past year will be recognized on the wall due to space constraints. Additional stars may be purchased for $100 each.

All funds received will support the Walkerton & District Hospital Foundation, specifically the Dr. RJ Creighton Family Birthing Centre.
